Advocacy
Pu‘ulu Lapa‘au's primary mission is to promote patient safety and care by improving the health, well-being and effectiveness of healthcare professionals. We do this by offering confidential support and advocacy to physicians, APRN's, physician assistants, nurses, dentists, psychologists, medical residents, medical students, veterinarians and other healthcare providers.
Provider health programs around the country have come a very long way in educating regulatory agencies, health organizations, medical insurance companies, medical liability companies and our community that many providers are able to maintain or regain their ability to safely function in the workplace. However, there are still significant minorities that continue to believe our organizations function without regard to the safety of patients.
Navigating these conflicting perceptions is always a challenge, which require communication and mutual trust between Pu‘ulu Lapa‘au and other organizations. We strive to establish and maintain good relationships with the other organizations. The mission of these very different organizations have a similar goal: to maintain the good health of our providers. A healthy provider is our best resource for the care of Hawai‘i's community!
Pu‘ulu Lapa‘au is able to advocate for healthcare professionals through our ability to provide assurance that a healthcare worker is able to maintain, or regain, safe practice. This often starts with an evaluation by a clinical team that is skilled in evaluation, diagnosis and management of provider health issues. Ongoing monitoring provides measurable data to demonstrate compliance with all monitoring requirements.

We sponsor workshops and presentations by national experts on the subject of wellness and community leadership.

Information and Consultation
Pu'ulu Lapa'au serves as the primary resource of information related to healthcare provider health in the state of Hawai‘i. Our directors and staff are available to respond to both general inquiries and specific requests for more information about our program.
We provide consultation to individuals, healthcare organizations and hospitals who may be concerned about the well-being, health, or functioning of a Hawai‘i healthcare provider.
We provide individual screening for self-referred providers, as well as those referred by friends, family, colleagues and employers. We will speak with any provider at their request, at any time, with full confidentiality.
Screening consists of listening to a provider's concerns, reviewing any reports, if applicable and outlining potential next steps. Engagement with us is strictly voluntary.
Monitoring
Monitoring is an essential element to clearly document the status of a provider's compliance, and it has been proven effective for enhancing long-term success. In fact, providers who are engaged in monitoring through health programs have the highest rate of recovery/abstinence as well as the ability to return to work.
Typical requirements of Monitoring Agreement may include:
- Regularly scheduled meetings with the Medical Director or the Board of Directors.
- Drug screening.
- Attendance with mutual support groups, such as 12-Step meetings.
- Workplace support monitors (usually a supervisor or similar).
The duration of a Monitoring Agreement varies according to the initial condition, or the reason for monitoring. Monitoring requirements tend to decrease over time, but is largely dependent on compliance.
Joint Commission Requirements
In 2001, The Joint Commission issued Requirement MS.11.01.01, which mandates hospitals and other health organizations to design a process that provides education about physician health; address prevention of physical, psychiatric, or emotional illness; and facilitates confidential diagnosis, treatment and rehabilitation of healthcare providers who suffer from a potentially impairing condition.
Further, MS.11.01.01 states "...the purpose of this process is to facilitate rehabilitation, rather than discipline, by assisting a practitioner to retain and or regain optimal professional functioning that is consistent with the protection of patients. If at any time during the diagnosis, treatment, or rehabilitation phase of the process it is determined that a practitioner is unable to safely perform the privileges he or she has been granted, the matter is forwarded for appropriate corrective action that includes strict adherence to any state or federally mandated reporting requirements."
Most healthcare facilities or organizations lack the resources and/or expertise to follow the strict guidelines set forth by MS.11.01.01. Moreover, maintaining confidentiality within a specific healthcare facility is difficult, at best, but is also part of the Requirement. The Joint Commission recognized that meeting the requirements could be difficult to fulfill, so they accept delegation of these duties to existing external programs to meet these standards.
Pu‘ulu Lapa‘au's is able to meet the Joint Commission requirements outlined in MS.11.01.01. Pu‘ulu Lapa‘au has been serving in this role for many healthcare organizations in the State of Hawai‘i.
